Obituary:
Hazel Ann Fulks, 79, of Ukiah passed away on Saturday, February 13, 2010 at her home with her family and friends present. Hazel was born October 5, 1930 in West Alexander, Pennsylvania; she had lived in Ukiah for the past 45 years.
Hazel was the only one of her brothers and sisters to have graduated from high school she also took ongoing classes to keep up her dietary certification she worked as a Dietician and head cook at Ukiah Valley Medical Center.
Hazel was proud of her 25 years of employment for Ukiah General Hospital and Ukiah Valley Medical Center her children, grandchildren and great grandchildren.
She was a leader for the Ukiah 4-H club in cooking and gardening and was active with Jobs Daughter with her daughter and with her sons in Demolay.
Hazel will remembered for her fine cooking, sense of humor, tenderness to her grandchildren, love for family and friends, love of opera, loved watching ice skating and birds and weekends to the coast. Her favorite singers were Perry Como and Dean Martin.
On November 19, 1949, Hazel married her best friend and soul mate and began lifes forty-five year journey.
She was a member of the Ukiah First Christian Church since 1962.
Hazel is survived by her daughters Shirley Fulks of Ukiah and Barbara Duval of Oregon, sons Phillip Fulks of Concord and Dennis Fulks of Ukiah, grandchildren Alexander Fulks, Soniamarie Fulks, Patrick Fulks, Michael Fulks, Rebecca Carvayo, John Robert Fulks, Amber Duval, Kandra Duval, Samantha Duval and Cassavndra Duval, great grandchildren Katelyn and Christian, also numerous nieces and nephews and cousins. Hazel was preceded in death by her husband John Wayne Fulks, parents Edgar and Hazel Miller, brothers Raymond M., James E. and Edgar R. Miller, sisters Ruby P. Miller, Thelma D. Smith and Opal Smith.
Funeral Services will be held Friday, February 19, 2010 at 11 AM at the Eversole Mortuary.
Memorial donations in Hazels name may be to the Ukiah Senior Center or the First Christian Church of Ukiah.
Arrangements are under the direction of the Eversole Mortuary.
